2025 Financial Assistance – Camp Woodstock gives out over $100,000 in scholarships every summer! We want as many campers to attend camp as possible. Don’t let money be an obstacle, the application is now online within the registration portal.

Medical Paperwork:
Online Health Form Instructions – Our health form is now online and can be completed with your camper registration. Here are instructions about how to log in and complete the health form.
How To Upload Medical Forms – You can upload medical forms directly into your campers registration. Here are the instructions about how to log in and upload the forms.
Health Assessment Record – Must be dated no more than 24 months prior to your camper’s final day at camp. The physical examination or health status certification shall include a statement about the child’s general health and the presence of any known medical or emotional illness or disorder that poses a risk to other children or which affects the child’s functional ability to participate safety in camp.
Immunization Record – Campers who are school age or younger shall include documentation that they have been adequately immunized according to the schedule adopted by the Commissioner of Public Health pursuant to section 19a-7f of the Connecticut General Statues.
Health Insurance Card – Camp needs a copy of the front and back of the insurance card to be uploaded to your camper’s registration portal, in addition to the information written on the medical form itself. All camper’s are required to have medical insurance valid for the duration of their stay.
THIS ONLY APPLIES IF YOUR CAMPER TAKES MEDICATION, PRESCRIPTION OR OVER THE COUNTER
One medication form is required for each over the counter and prescription medication brought to camp. Each form must be signed by your child’s healthcare provider. All medications must be in its original container. The form and medication will be collected at check-in and returned at check-out.
Health Care Plan –
THIS ONLY APPLIES IF YOUR CAMPER HAS A DISABILITY OR SPECIAL HEALTH CARE NEED SUCH AS ALLERGIES WITH EPI PENS, SEIZURES, ETC.
If your camper has a disability or special health care needs, an individual care of plan needs to be developed with the child’s parent(s) and healthcare provider. The plan shall include appropriate care of the child in the event of a medical or other emergency. Please use this form to develop your individual care plan with your child’s healthcare professional. If you have any questions about camp’s health management practices and the support your child may need, please contact camp staff at 860-974-1336.
